Health Pay: $1,872 to $2,016 per week Shift Information: Days - 3
days x 12 hours Contract Duration: 13 Weeks Start Date: ASAP About
the Position Pay Rate: $52.00 - $56.00/Hourly. Summary: We are
seeking a licensed occupational therapist to join our growing team
of medical professionals. You will be working with patients who are
recovering from injuries and disabilities and help them regain
their abilities so that they can resume work. Duties will include
conducting a physical assessment of clients, developing treatment
plans, assessing work and home environments of clients, and
developing physical rehabilitation programs. Responsibilities:
Evaluate and treat patients in the acute care and nursing home
settings. Also screen patients and attend meetings regarding
patient progress. Documentation of evaluation, progress notes,
daily notes, insurance, and discharge notes in patient's medical
records. Collaborate with team members to ensure effective
carryover of services for patient care. Outpatient evaluation and
treatments. Perform other duties as requested. Requirements
Requirements: 1 - Year experience Required, National Board
Certification in Occupational Therapy and Wisconsin occupational
therapy license and continuing education to maintain licensure,
Must maintain a valid, current driver’s license, Master of
Occupational Therapy required. Doctorate preferred, BLS
Certification Occupational Therapy License. Benefits Benefits:
