Job Description:

seeking a passionate and dedicated Occupational Therapist to
deliver high-quality care tailored to patients in a home setting,
fostering independence and improving their daily living skills.
This position offers the unique opportunity to develop
individualized treatment plans and work one-on-one with patients to
achieve their highest possible levels of function and wellness.
Evaluate patients’ functional statuses and develop individualized
treatment plans. Implement therapeutic strategies to enhance
patients’ abilities to perform daily activities. Document patient
care and progress collaboratively within a multidisciplinary team.
Educate family members and caregivers on treatment plans to ensure
a supportive environment. Maintain patient confidentiality and
adherence to professional standards. Strong interpersonal and
communication skills. Ability to work independently in a home
health setting. Proficient in using healthcare documentation and
reporting software. Medical Insurance Dental Insurance Vision
Insurance 401k Valid Occupational Therapist license in the state of
practice. NBCOT Certification.
