• Provide high-quality patient care in a Long Term Care environment
• Collaborate with healthcare teams to develop and implement individualized care plans
• Conduct thorough assessments and evaluations to identify patient needs
• Develop and implement effective treatment plans to promote patient recovery and well-being
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordinationRequirements:
  • Active WI Registered Nurse, Long Term Care (LTC) license
  • 2+ years of Long Term Care/Skilled Nursing Facility - LTC/SNF experienceBenefits:
