Job Description:

The Hospice NP will provide care for Gundersen Health System Hospice patients in collaboration with the Hospice Medical Director, as well as other providers and members of the care team. They will perform patient exams, obtain histories, develop assessments and plans, and implement appropriate plans of care. Primary focus is placed on the quality of life and interdisciplinary needs of patients and families who are faced with progressive disease and end of life processes. Care is provided to patients wherever they are, i.e. hospital, clinic, nursing home, and home settings. The NP is an educator within the organization regarding pain and symptom management and the support of patients/families at the end of life. Patient and family education and counseling are also inherent in hospice care. Essential Functions and Responsibilities of the Job

• Assesses present and potential needs of patient and family
• Review Plan of Care with patient, family, physician, IDG and evaluate and/or modify as needed
• Provide leadership in care coordination at IDG, demonstrating expertise in the function of a Nurse Practitioner
• Complete all Face to Face and Recertification visits as well as documentation in a timely manner per Medicare standards
• Manage symptoms and teach patient/family care management
• Follow company policies, procedures, and directives
• Interact in a positive and constructive manner
• Prioritize and multitask
• Provide leadership in care coordination, communication to all team
• Attend IDG meetings regularly with face-to-face documentation completed
• Demonstrate continuous development of clinical expertise and current knowledge of hospice processes
• Demonstrate expertise in the function of a nurse practitioner
• Supervise and teach patient and care providers in disease process, treatment plan, and comfort measures. Evaluate patient, family, and caregiver comprehension and response to treatment plan, and modify as needed
• Provide direct nursing measures, appropriate, effective symptom management, documents interventions, including medication orders and physician interaction as appropriate
• Document pertinent information clearly, accurately, and promptly, according to Hospice standards
• Communicate with Case Manager prior to IDG meeting for recertifications --This position is open to Clinic Nurse Specialists, Nurse Practitioners who have appropriate training and experience. This position will be outpatient only with the schedule being Monday thru Friday. There are no scheduled night shifts however occasional nighttime work can be expected depending on various pain management and end of life care needs. This position will require a Outpatient Palliative/Hospice call rotation. --Work Experience: - Required: 3+ years in clinical nursing or direct patient care, preferably in acute care settings, oncology, med-surg, or another related field. - Desired: Experience in acute and chronic pain management as well as research, data collection and analysis. --Here are some highlights:
